What should I do first when an injury occurs?
Render first aid to the injured employee or if the injury is of an “emergency” nature, seek immediate qualified medical attention and the nearest location.
If the injury is not of an urgent nature the employee needs to pick a doctor off the PANEL OF PHYSICIANS, normally an urgent care center or doctor’s office.
TELL THE HOSPITAL OR DOCTOR THAT THE INJURY IS WORK RELATED.
If possible, send the attached two forms with the claimant to the doctor/hospital. The first form (click here for copy) is authorization to treat which the employer needs to fill out. The second is the pharmacy drug information sheet for the employee.
Have the employee complete the Employee’s Statement of Injury form (click here for copy) as soon as possible. This form should be completed by the injured employee and signed. Forward all additional information regarding the injury (medical bills and reports, employee work status reports, legal forms, court orders, etc.) directly to United Business Insurance Co.
We will contact you within 24 hours of the claim being reported.
